We are seeking a Caregiver Support Specialist that possesses exceptional technology, organizational, communication, and time management skills, and experience working in a fast-paced environment.

SUMMARY

The Caregiver Support Specialist provides care consultations, educational seminars, and resources to both caregivers and older adults living independently (solo-agers). This role includes linking clients to essential services internally and externally and facilitating a weekly support group.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Case Management
• Effectively manages an ongoing caseload of family caregivers and solo agers
• Conducts thorough care consultations with family caregivers and solo agers
• Responsible for follow-up activity related to all cases within your assigned caseload
• Maintains detailed documentation on client activity in Sales Force
• Provides coverage for essential casework activity in the absence of other Caregiver Support Specialists
• Familiarizes self with both local and national resources
• Lead support group in the absence of support group facilitator

Education
• Helps develop appropriate program content for targeted audiences
• Conduct education presentations to clients, community organizations, centers of faith, and businesses

Outreach
• Outreach to various diverse communities to increase client base
• Manage vendor tables at community events.
• Builds relationships with professional organizations to further expand program.
• Collaborate with CSP (Caregiver Support Program) Director to create opportunities for presentations, forums, and community events
• Represents the agency at meetings in the community, as needed

Administrative
• Assists director with the outcome evaluation of the program
• Assists in the planning and implementation of special seminars, community outreach, and resource days
• Assist support group facilitators with monthly meetings
• Document all program activity client cases, follow-up calls, support group and education class attendance in Salesforce database

General
• Attend regular agency staff meetings, component staff meetings and in-services training courses
• Performs other duties that may be assigned

KEY TECHNICAL SKILLS AND KNOWLEDGE

To perform this job successfully, an individual must be able to perform each qualification satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

• Work compassionately and effectively with older adults, family caregivers, colleagues, and other professionals.
• Great interpersonal and communication skills
• Culturally competent with strong understanding of the intersectionality of racism and poverty
• Excellent technology skills, with proficiency in Salesforce or another CRM (Customer Relationship Management) platform required
• Ability to carry out assignments and projects without detailed instructions
• Ability to thrive in a fast-paced, deadline-driven environment

EDUCATION/EXPERIENCE
• A master’s degree in social work or related field of study from an accredited university, a licensed master level social worker preferred
• A minimum of a bachelor’s degree in a human services field and at least 2 years’ experience in aging and/or social services with experience working in a direct service capacity may be substituted for a master’s degree
