Long-Term Care Ombudsman

Reports To: Long-Term Care Ombudsman Program Director

Status: Full-time; Exempt

Purpose:

The Long-Term Care Ombudsman Program protects the health, safety, welfare and rights of Dallas County nursing home and assisted living residents by advocating for their quality of life and care. The Long-Term Care Ombudsman serves the residents in assigned nursing homes and assisted living facilities, by monitoring care provided in the facilities and working to resolve complaints on behalf of the residents. 

Primary Responsibilities: 

  • Engaging in all aspects of the general operation of the program and maintaining the program in the absence of the director, as assigned.
  • Scheduling and making regular visits to assigned nursing homes and assisted living facilities.  
  • Receiving, investigating and working towards resolution of complaints reported to the Ombudsman Program.
  • Being knowledgeable of the licensing standards for nursing homes and assisted living facilities.
  • Providing guidance on the process of choosing a facility for persons in the community and sharing information on specific facilities. 
  • Scheduling and making presentations in the community, as well as to facility staff, family groups and resident councils to increase visibility of the Ombudsman Program.
  • Providing one-on-one consultations on long-term care issues for residents, families and facility staff.    
  • Attending Texas Health & Human Services Commission, Long-Term Care Regulation surveys and exits, as notified.
  • Assisting in the training, supervision, retention and evaluation of volunteer ombudsmen who are assigned to specific nursing homes and assisted living facilities.  
  • Maintaining thorough documentation of visits and casework, preparing monthly reports, and working closely with the Long-Term Care Ombudsman Program Director to monitor progress towards program goals.
  • Developing a working knowledge of, and remaining current on, local, state and national issues involving long-term care facilities, and engaging in systems advocacy related to those issues, when appropriate.

Qualifications:  

Education/Experience:

A bachelor’s degree in social work or a related field with a minimum of two years of experience in social services, preferably in the field of aging or advocacy.  

Skills/Characteristics:

Bilingual required. Demonstrate diplomacy, assertiveness and problem-solving skills; work effectively with individuals of varying backgrounds, ages and interests; and possess strong public speaking and writing skills. Excellent organization and computer proficiency required, including proficiency with Salesforce, PowerPoint, Excel, and Word. Knowledge of aging preferred.

Guardianship Case Manager

Position Title:  Guardianship Case Manager
Reports To:     Guardianship Program Director
Status:              Full Time and Exempt

Purpose:  The Guardianship Case Manager supports vulnerable older adults who have been determined by the courts to need guardianship protection by ensuring all health and well-being needs of clients are met.

Primary Responsibilities:

 

  • Complex case management for older adults with cognitive impairments that ensures their health, safety and well-being
  • Monthly face to face visits (reliable transportation required)
  • On call 24/7 for client emergencies
  • Make all decisions for guardianship clients in a client-centered manner by encouraging self-determination, by use of substituted judgment or best interest standards, as appropriate
  • Complete documentation professionally and within required time frames
  • Assist with program activities including benefits coordination and volunteer training
  • Ability to pass required background checks and professional certification exam

 Qualifications:

  • Bachelor’s or Master’s degree in social work from an accredited college or university and a minimum of one year of full time experience (with Master’s degree) or two years of full time experience (with Bachelor’s degree) in direct social service work. (LMSW preferred)
  • Ability to pass a state certification test and become a certified guardian within a year of hire date.
  • Experience working older adults, and experience working with volunteers, a plus.
  • Strong organizational skills and attention to detail.
  • Strong interpersonal and communication skills with a desire for continuous learning.
  • Excellent technology skills, with proficiency in Salesforce or other CRM platform required.
  • Ability to carry out assignments and projects without detailed instructions.
  • Ability to thrive in a fast-paced, deadline-driven environment.
  • Bilingual in English and Spanish preferred.

Chief Development Officer

PURPOSE OF POSITION:

The Chief Development Officer leads The Senior Source’s contributed revenue efforts. The position reports to and works intimately with the President & Chief Executive Officer and will assume the Agency leadership role in fundraising with a staff of three direct reports.  The ideal candidate is growth and success-oriented with a proven track record in event, major gifts and annual fundraising campaigns.  The candidate demonstrates the ability to organize a dynamic constituency toward a common goal. Succeeding in this environment will require strategic and visionary thinking while at the same time, taking a hands-on approach to connecting with a broad array of supporters using traditional and non-traditional development methods.

JOB RESPONSIBILITIES:

  • Develop strong relationships with, and secure the trust and confidence of the CEO, key funders, board members, staff, community members, and others critical to the agency’s development efforts.
  • Play a hands-on, creative leadership role in the identification, cultivation and solicitation of major individual and institutional gifts; personally cultivate and solicit a portfolio of donors and prospects.
  • Develop and execute on The Senior Source’s Strategic plan, specifically taking ownership and accountability for the development goals and activities within the plan.
  • Set annual plans and goals for the agency’s fundraising strategies. Establish budgets and track and report on progress.
  • Provide staff leadership and partnership for the development committees of the Board of Directors.
  • Manage the team of three; assess and evaluate the success of the individual team members and the department as a whole; of; ensures that the department is well-structured and able to support annual goals.
  • Works closely with the Director of Marketing & Communications to create fundraising collateral, event branding and donor relations communications.
  • Represents the organization externally with prospects and donors
  • Other activities as requested by the President & CEO.

QUALIFICATIONS:

Development:

  • Strong track record of developing and executing against annual plans and goals while managing budgets and performance
  • Proven ability to develop excellent rapport and work effectively and confidentially with donors, board members and key community leaders and to interact with them on a professional and social basis with ease and comfort
  • History of cultivating and securing large annual and multi-year gifts
  • Experience in relationship management and database software is required, ideally Raiser’s Edge.

Leadership:

  • A bachelor’s degree is required and ideally 10 years’ experience in nonprofit leadership
  • History of making decisions in a changing environment and anticipating future needs.
  • Ability to remain focused and organized under multiple and tight deadlines.
  • Excellent oral and written communication skills, along with customer service and team skills, are required.
  • People management experience required with a strong emphasis on building teams.
  • Strong organizational skills, attention to detail and time management capabilities.

Purpose:

The Director of Corporate and Foundation Relations reports to the Chief Development Officer and is a member of the organization’s Development team. The Director is responsible for strategic design, execution, management and measurement of The Senior Source’s non- government grants programs, with the goal of increasing contributed revenue and donor engagement.

Primary Responsibilities:

  • Research potential grant funding sources (corporations, foundations, organizations,) and collect data to analyze gift potential.
  • Develop and maintain a timeline for grant
  • Work collaboratively with the CDO, COO and program directors to understand program goals and priorities to write meaningful proposals and to develop specific requests as grant opportunities become available.
  • Write and submit proposals, applications, and follow up Participate in solicitation and cultivation meetings.
  • Maintain grant files and
  • Utilize Raiser’s Edge to track grants, produce contact notes, and conduct appropriate follow-up.
  • Develop a portfolio of reports from the Raiser’s Edge data base for measuring

performance, analysis, and reporting.

Education/Experience:

  • Bachelor’s degree required
  • 5-7 years of Development experience including demonstrated success in grant writing, preferably in a non-profit community-based organization.
  • Proven success in identifying and soliciting funds from individuals, corporations, and foundations from a variety of sources and in developing relationships with
  • Exceptional written and verbal communication skills required. Demonstrated ability to write clear and persuasive proposals.
  • Excellent time management skills and a high degree of organization, careful attention to detail, ability to multitask and to prioritize and manage multiple projects and deadlines in a fast-paced environment.
  • Knowledge and experience of data entry systems and reporting (Raiser’s Edge experience preferred).
    Well-disciplined, self-motivated and goal-driven; able to work with a high degree of independence as well as part of a team.
  • Ability to appropriately handle confidential matters
  • Proficient in the use of MS Office programs
  • Ability to work on infrequent nights and weekend evenings as required by development department events.

Position Overview: 

The ideal Marketing Coordinator candidate will join a growing, dynamic marketing team of three to provide agency support with thoughtful concept creation, design, implementation, and promotion of agency programs and events. This individual is primarily responsible for coordinating and creating content for all channels across digital and print media. 

 

Primary Responsibilities: 

  • Collaborate with staff to obtain information for regularly scheduled newsletters for programs 
  • Execute the social media content strategy that is aligned with short-term and long-term marketing goals established by the Director of Marketing 
  • Maintain the agency event calendar and ensure timely updates are made 
  • Coordinate with program staff and Content Managers to acquire information in advance to promote events for clients. 
  • Understand The Senior Source’s brand voice and positioning to reach our target audiences through a variety of social channels 
  • Analyze monthly metrics of paid social vs organic social and work with the Director of Marketing to refine as needed to achieve optimal audience reach.   
  • Keep current and stay on top of senior-related news trends and articles  

 

 

Qualifications: 

  • 4-year college degree is required; preference in marketing or communication
  • 1-3 years experience in related field 
  • Proficiency in Adobe Creative Suite applications  
  • Experience in MailChimp 
  • Video creation and infographic experience is a plus 
  • Excellent verbal and written communication skills 
  • Proficiency in Microsoft Suite
