POSITION TITLE: Guardianship Case Manager
REPORTS TO: Guardianship Program Director
STATUS: Full Time; Exempt
Join the team at The Senior Source and feel the energy, enthusiasm, and passion for Making Older Better. We are seeking a Guardianship Case Manager with experience working with older adults and that possesses exceptional interpersonal and communication skills.
PURPOSE:
The Guardianship Case Manager supports vulnerable older adults who have been determined by the courts to need guardianship protection by ensuring all health and well-being needs of clients are met.
PRIMARY RESPONSIBILITIES:
• Complex case management for older adults with cognitive impairments that ensures their health, safety and well-being
• Monthly face to face visits across the North Dallas County area (reliable transportation required)
• On call 24/7 for client emergencies
• Make all decisions for guardianship clients in a client-centered manner by encouraging self-determination, by use of substituted judgment or best interest standards, as appropriate
• Complete documentation professionally and within required time frames
• Assist with program activities including benefits coordination and volunteer training
• Ability to pass required background checks and professional certification exam
QUALIFICATIONS INCLUDE:
• Bachelor’s or Master’s degree in social work from an accredited college or university and a minimum of one year of full time experience (with Master’s degree) or two years of full time experience (with Bachelor’s degree) in direct social service work. (LMSW preferred)
• Ability to pass a state certification test and become a certified guardian within a year of hire date
• Experience working with older adults, and experience working with volunteers, a plus
• Strong organizational skills and attention to detail
• Strong interpersonal and communication skills with a desire for continuous learning
• Excellent technology skills, with proficiency in Salesforce or other CRM platform required
• Ability to carry out assignments and projects without detailed instructions
• Ability to thrive in a fast-paced, deadline-driven environment
• Bilingual in English and Spanish preferred
