- Photo ID
- Social Security Card
- Birth certificate/Naturalization document
- Current income (Social Security, pension, or pay stubs)
- Current bank statement for all accounts (two most recent would be ideal)
- Medical bills/Pharmacy receipts
- Lease/Mortgage statements
- Recent utility bills with current address
- Property tax record
