Benefits Enrollment Clinic

Attend this clinic to apply for state benefits (SNAP, Medicaid Health Insurance, Extra Help with Prescriptions, and Medicare Savings). RSVP to EFSC@theseniorsource.org

Items to bring:

• Photo ID
• Social Security Card
• Birth certificate/Naturalization document
• Current income (Social Security, pension, or pay stubs)
• Current bank statement for all accounts (two most recent would be ideal)
• Medical bills/Pharmacy receipts
• Lease/Mortgage statements
• Recent utility bills with current address
• Property tax record
